Request Help for a Foot or Ankle Concern

Use this page when pain, swelling, a nail problem, a wound, diabetic foot risk, or a recurring sports injury is making it harder to walk, work, exercise, or wear normal shoes.

What This Request Is For

Patients do not need to know the exact diagnosis before contacting the clinic. It is enough to explain the symptom and how it affects daily life. Stamford Podiatry Group can review the concern and help patients move toward the right type of visit.

If the problem has lasted, keeps coming back, or changes how you walk, a podiatry visit can help sort out whether the cause is soft tissue strain, pressure, a nail or skin issue, diabetic foot risk, injury, joint pain, or a condition that may need imaging or follow-up.

Common Reasons Patients Request Care

  • Heel pain, arch pain, or plantar fasciitis symptoms.
  • Painful toenails, ingrown toenails, thick nails, corns, calluses, or skin concerns.
  • Ankle pain, sprains, sports injuries, tendon pain, or recurring activity pain.
  • Diabetic foot checks, wounds, ulcers, pressure areas, numbness, or circulation concerns.
  • Bunions, hammertoes, flat feet, orthotics questions, or foot surgery questions.

After You Send the Form

The appointment request gives the clinic a starting point. If the concern involves diabetes, wounds, signs of infection, injury, or worsening pain, include that clearly so the team understands the situation.

Before Your Visit

Bring your insurance card, photo ID, medication list, and the shoes you wear most often. If you have recent imaging, records, or notes from another provider, bring those too.
